To join the Polish Aligner Academy, follow the steps below:

• Zapoznać się ze statutem stowarzyszenia.
• Know or get to know two members who will advise whether it is worth joining us, and who can support your introduction to the association with their signature.
• The form can be filled out electronically and signed by hand, for example, with a pen.
• Members must sign the form by hand. (The introducer’s name field must also be completed). Save and send it to us via email.
• The easiest way to do this is on paper at any event.
• Alternatively, you can print the form, fill it out on paper, sign it and scan or take a photo of the document. Please have legible scans or photographs, matching the quality of official documents.
